INSTRUCTIONS:  Open this Modification Request Form, print, fill out completely, and mail to the Board in DUPLICATE   or
                              print this web page, fill out completely, and mail to the Board in DUPLICATE.
MODIFICATION REQUEST REQUIRED:   Residents are required by the CC&R's to submit a “Modification Request” for approval by the Board of Directors before you do any exterior modifications to your property.  This includes home painting, structural additions, fences, sheds, pools, etc.
CC&R:   Review the covenants in your copy of the CC&R's or use  the CC&R's online. 
 ALLOW 2 WEEKS FROM RECEIPT FOR REVIEW:   Your modification request will be reviewed by the Architectural Control Committee (ACC).  Please allow at least two weeks from date of receipt of your modification request for this review. If the ACC has any questions on your request, they will contact you.
 APPROVAL OF MOD REQUESTS EXPIRE AFTER 6 MONTHS: HOA's Modification Requests approval expire after six (6) months. Approved work not completed within 6 months must have a new Mod Request submitted for new HOA approval.

For color changes and HOA approval, the homeowner must paint a sample of the color on the home and then request an inspection by the ACC for paint approval.  This request form and a 4" x 4" color sample is still required to be submitted.
